Your Program is now active and available for you to Activate your account with Affinity.

To do so, please click the Activate account button on the bottom right of the screen. Please ensure that you have activated your account within the allotted time period, using your Affinity PIN # provided in your Welcome Package. 

Follow the directions in your Affinity Welcome Package to ensure compliance with your HIMS Program. Once you have activated and selected your username and password, you will be able to Login to your personal compliance page.
