Welcome!
Your Program (with the Kentucky Board
of Nursing) is now active and available for you to activate your account with
Affinity. Please ensure that you have
activated your account within the timeframe specified to ensure compliance with
the Program. To do so, please click the
Activate account button on the bottom right of the screen. Once you have activated your account and
selected a username and password, you will be able to login to your personal
compliance page.
You are required check-in daily, seven
(7) days per week between the hours of 5:00 am and 1:00 pm. You may check-in by phone or on-line. Should you fail to check-in as required, you
will be considered non-compliant with the Program.
You are required to test on the day you are selected. If you are a no-show for any reason, you will be considered non-compliant with the Program.
You are required to maintain
sufficient funds in your account for testing purposes. Maintaining a balance in your account will
enable you to process your test details and obtain an authorization code to
test. Having insufficient funds and/or
failure to obtain funds when selected to test will result in a no-show and you
will be considered non-compliant with the Program.
If you are locked out of your account
for any reason, you will be considered non-compliant with your Program.
You are required to keep an adequate
amount of chain of custody forms on-hand at all times. If you are low (four (4) or fewer forms) on
chain of custody forms, order more forms.
Not having a chain of custody form on-hand when you have been selected
to test may result in a no-show.
You are required to keep your personal
information up-to-date with Affinity and with the Program. Please ensure your personal compliance page
under the Personal Info Tab (Demographics) is correct at all times to ensure
effective communication.
