Dear Ridgeview Institute Participant:

Welcome to the Ridgeview Institute Professional Aftercare Program. Ridgeview Institute has entered into an agreement with Affinity to provide confidential drug testing services and compliance data administration services. Your Program Coordinator, has provided your name and some demographic confidential information to begin your participation in this comprehensive administration and monitoring program.

The goal is to provide you with convenient validation tools to document your compliance, as compliance is the cornerstone of what we do. It allows us to have credibility with the regulatory Boards and effectively advocate for your return to practice if applicable. You are required to remain active and compliant with Affinity, no exceptions.

Please “click” the Activate account button on the bottom right of the screen. This is necessary to ensure compliance with your Check-In requirements. Failure to do so could result in a Missed Check-In. Once you have activated and selected Username and Password, you will be able to Login to your personal compliance page.

Check-In is required Monday through Friday, from 3:00 am until 3:00 pm. After 3:00 pm, your “missed” Check-In requires that you “Self-Test”, same day. You may Check-In by telephone or online. Your program prefers that you use online Check-In.

Remember to remain abstinent from “all” mood-altering substances. Be careful of over-the-counter medications. Please consult your Psychiatrist or Addictionologist.
