Your Program is now active and available for you to Activate your account with Affinity.
To do so, please click the Activate account button on the bottom right of the screen. Please ensure that you have activated your account within the allotted time period, using your Affinity PIN # given to you by your Case Manager.
Follow the directions in your Affinity package to ensure compliance with your Check-In requirements. Failure to do so could result in a Missed Check-In. Once you have activated and selected your username and password you will be able to Login to your personal compliance page.
Your Check-In time is Monday to Sunday, 5:00 am - 5:00 pm. You may Check-In by phone or online.
