Your Program is now active and available for you to Activate your account with Affinity. To do so, please click the Activate account button on the bottom right of the screen. Please ensure that you have activated your account by Thursday May 31, 2012 to ensure compliance with your Check-In requirements. Failure to do so could result in a Missed Check-In. Once you have activated and selected username and password you will be able to Login to your personal compliance page.

Daily Check-In will begin Friday June 1, 2012.  You are required to Check-In and Test Monday-Sunday between 5:00 am and 5:00 pm. You can Check-In by phone or online.
